Main passport applicant
A. For the FIU Due Diligence checks:

A valid passport copy – notarized or certified by lawyer (must be valid for another 6 months beyond the citizenship application date)

A copy of your identity card – notarized or certified by lawyer (if you don’t have an identity card, a notarized or certified copy of your driver’s license will suffice also).

Your duly completed FIU application form

A police clearance certificate – no criminal record.
B. For the filing of your application:

A notarized copy of your birth certificate

Asset report – certified copy – prepared by an accountant, lawyer, notary public or an auditor – we can supply a sample on request

Medical examination certificate prepared by a physician – we can supply a sample on request

A scanned high-resolution color version of your passport photo

Your employment certification – document issued by your employer, or an affidavit if you’re self-employed – a sample can be provided on request

Bank reference – issued by your banker – a sample can be provided on request

Professional reference – document issued by your lawyer/accountant or another professional who knows you

A copy of your academic qualification

Proof of address/utility bill

Nomination Form – a sample can be supplied on request

Spouse/Adult dependent applicants:
